I play through on Normal my first runthrough, then if the game is good enough to warrant another runthrough for extras/achievements/different endings, then I'll go the hardest possible.

With endermen? zombies? passive agressive spiders? those shitty silverfish things? I am constantly on Peaceful on my Dirt Castle world, but, on my creative mode world I like having it on hardcore just to screw with the creepers.
