You have right in most things. Just Rare didn't have plans for Code Chamber codes. They only thought that secret items would unlock stuff which aren't need to complete BT. Later it became as Breegull Bash, homing eggs, Jinjo as multiplayer character and Dragon Kazooie.

They had the idea for it, but they hadn't gotten as far as to program them when they had to pull the plug on Stop'n'Swop.

They only had plan that lever in DK64 would break icy wall in BK. Then ice key would open new area in DK64.

Which I can only assume happened before BK was released, given the current evidence. However, the fact that DK64 Kiosk is missing the "ice-key" text is a little suspicious. Then again, most of the other in-game menu items are missing as well. Therefore, I'll need to figure out whether they were removed(for whatever reason) or just not programmed yet. If the latter is true, I can safely say that Rare was messing around with the Ice-key's functionality in DK64 long after BK's release.
